COLLABORATIVE FORMS
Complete these steps to register as a collaborative student:
- Complete the Collaborative Student Registration Form.
- After BSC receives your collaborative registration form, we will check to make sure that you are eligible to take these courses collaboratively.
- BSC will then give the provider campus permission to register you for the classes you requested.
- You will receive a confirmation email within 2 weeks after the registration has been completed.

Complete these steps to withdraw/drop as a collaborative student:
- Complete the Collaborative Withdrawal/Drop Request Form.
- After BSC receives your request, we will forward it to your provider college.
- You will receive a confirmation email within 2 weeks after withdrawal/drop has been completed.
