HOW TO APPLY FOR DISABILITY SERVICES
- The student is responsible to initiate a request for accommodations by applying for Student Accessibility (SA). An application can be requested either in person, by telephone, or Complete and Submit the Application for Student Accessibility Form. In order to receive accommodations, the student is required to provide current documentation (view GUIDELINES FOR DISABILITY DOCUMENTATION (Appendix B) of the disability as well as the need for reasonable accommodation. In requesting accommodations that require time to prepare, such as electronic textbooks or hiring sign language interpreters, requests need to be made early enough to allow ample time to fulfill the request, whenever possible, 60 days before the beginning of the semester.
- The student meets with the SA coordinator to do the following:
a. Provide initial information about him/herself.
b. Review documentation.
c. Discuss needed accommodations.
d. Discuss policies and procedures.
e. Complete appropriate accommodation forms for faculty.
- The SA coordinator will provide letters to instructors verifying a disability and stating the reasonable and appropriate accommodations recommended by the Student Accessibility office. The student is responsible for delivering the letter to the instructor as soon as possible at the beginning of the semester.
Please contact BSC's Office of Financial Aid in person at Schafer Hall 131 or by phone at (701)224-5494 or (800)445-5073 regarding availability of funding and to obtain a financial aid application. The Student Accessibility Office does not provide financial assistance.
Check with your regional Vocational Rehabilitation (VR) to see if you are eligible for services. BSC works with VR offices to serve you.

GUIDELINES FOR DISABILITY DOCUMENTATION
Documentation should indicate how the disability substantially limits the student in a major life activity. The information submitted is used to determine appropriate and reasonable accommodations for students with disabilities in the North Dakota University System.
Documentation provided by the student should include the following:
- A diagnostic statement identifying the disability, date of the current diagnostic evaluation and the date of the original diagnosis;
- A description of diagnostic criteria:
- A description of how the disability currently affects the student in the learning environment, e.g., functional limitations;
- Treatment, medications, assistive devices/services currently prescribed/used;
- A description of the expected progression or stability of the impact of the disability over time;
- The relevant credentials of the diagnosing professional(s) such as medical specialty and professional licensure.
